I have a kotatsu in my Japanese house, but I still need to have warm air. So I also use an electric one-room heater/air conditioner. Other people use a kerosene heater for warmth (portable or built-in). Until it went out of business, the local gas station would send a truck around the neighborhood offering to sell kerosene in the cold months.
